Football Equipment Agreement

Chapman High School is intent on providing the best possible football experience to all in there District 1 area.  In doing so we have partnered with the Inman Youth Association to help provide and monitor the reconditioning of the helmets, shoulder pads and pants.  This equipment is property of Chapman High School and Spartanburg School District 1. 

In accepting the use of the equipment I agree to appropriately take care of and return the items as instructed. 

I also agree to accept the use of the equipment with the understanding that football is a dangerous sport and serious injury can occur even when properly outfitted. 


The Riddell Helmet Warning States: 

“NO HELMET CAN PREVENT SERIOUS HEAD OR NECK INJURIES"… contact in football may result in CONCUSSION BRAIN INJURY which no helmet can prevent. Symptoms include: loss of consciousness or memory, dizziness, headache, nausea, or confusion. If you have symptoms, immediately stop playing and report them to your coach, trainer, and parents. Do not return to a game or practice until all symptoms are gone and you have received MEDICAL CLEARANCE. Ignoring this warning may lead to another and more serious or fatal brain injury.

I have read the above information and warning statement and agree to the terms of equipment use.

I understand that if the equipment is lost, stolen or damaged that I will be held to monetary charges for the replacement of such items.  (Helmets $89.99, Shoulder Pads $49.99, Pants $29.99)

I hereby release, discharge and/or otherwise indemnify Chapman High School, Spartanburg School District 1, Inman Youth Association, it's sponsors, it's officers, it's coaches, it's volunteers, it's employees and associated personnel, including the owners of the fields and facilities utilized for the program, against any claim by or on behalf of registrants as a result of the registrant's participation in the program.
